Case Management
Assertive case management services are provided to help individuals achieve the objectives they have defined and reach their ultimate goal of self-sufficiency. Through required weekly meetings and an Individual Development Plan, guests receive individualized counseling and support in identifying, monitoring and achieving goals. In addition to this one-on-one work with guests, we link guests to services available in the community, including health care, housing, employment, educational opportunities and job-training programs. We also offer ongoing case management services to our alumni when they need it to ensure they have every possible opportunity to successfully manage their lives.

Monday: 9-11 a.m. & 2-3 p.m.
Tuesday: 9-11 a.m. & 2-3 p.m.
Wednesday: 9-11 a.m. & 2-3 p.m.
Friday: 9-11 a.m. & 2-3 p.m.
Thursday: 1-3 p.m.
